Possible HIV exposure के बाद सही next step सिर्फ “कितने दिन बाद test?” नहीं है। सबसे पहले यह देखना जरूरी है कि exposure कितना recent है, क्योंकि PEP की emergency window 72 hours होती है। उसके बाद test type के अनुसार सही testing timeline follow की जाती है।

Step 1: Exposure 72 hours के भीतर है?

अगर possible exposure पिछले 72 hours के भीतर हुआ है, तो PEP evaluation के लिए तुरंत qualified clinician/emergency service से contact करें। PEP जितना जल्दी शुरू हो उतना बेहतर है। HIV test result का इंतजार करके PEP window miss नहीं करनी चाहिए।

Step 2: Baseline testing

Clinician exposure के समय baseline HIV testing कर सकता है। Very early negative test उस recent exposure को rule out नहीं करता, क्योंकि कोई test infection के तुरंत बाद detect नहीं करता।

Step 3: Test type के अनुसार window period

  • NAT: लगभग 10–33 days.
  • Lab Ag/Ab from a vein: 18–45 days.
  • Rapid finger-stick Ag/Ab: 18–90 days.
  • Antibody test: 23–90 days.

ये CDC ranges हैं। Individual follow-up schedule test used, PEP/PrEP use और ongoing exposures के कारण अलग हो सकता है।

Step 4: Negative result के बाद क्या?

अगर test अपने window period के खत्म होने से पहले हुआ है, तो repeat testing की जरूरत हो सकती है। अगर window period के बाद test negative है और उस दौरान कोई नया exposure नहीं हुआ, तो clinician उसी test type के context में result interpret कर सकता है।

Symptoms का इंतजार क्यों नहीं करना चाहिए?

Acute HIV में symptoms हो भी सकते हैं और नहीं भी। Fever या rash HIV-specific नहीं हैं। Exposure-based testing plan symptoms से ज्यादा reliable है।

When to seek prompt or emergency care

A possible HIV exposure within the past 72 hours needs urgent PEP assessment—the sooner, the better. Seek emergency care for severe breathing difficulty, confusion, collapse or another acute medical crisis. Testing and treatment should remain private, non-judgmental and clinician-led.
