HIV prevention: PrEP, PEP, U=U and safer practices

HIV prevention अब सिर्फ एक method पर depend नहीं करती। आज PrEP, PEP, effective ART/U=U, condoms, sterile injection equipment और regular testing जैसे multiple evidence-based tools available हैं। सही option व्यक्ति की situation पर depend करता है।

PrEP क्या है?

PrEP (Pre-Exposure Prophylaxis) HIV-negative लोगों के लिए prevention medicine है जिन्हें HIV exposure हो सकता है। PrEP pills और long-acting injectable options उपलब्ध हैं; कौन सा option suitable है, यह medical history, preferences और access के अनुसार clinician के साथ तय किया जाता है।

PrEP HIV prevention के लिए highly effective है जब prescribed तरीके से use की जाए। यह other STIs या pregnancy को prevent नहीं करती।

PEP क्या है?

PEP (Post-Exposure Prophylaxis) possible HIV exposure के बाद emergency prevention है। इसे 72 hours के भीतर शुरू करना जरूरी है और जितना जल्दी शुरू हो उतना बेहतर है। PEP routine prevention का substitute नहीं है; frequent/recurrent risk में clinician PrEP discuss कर सकता है।

U=U भी prevention है

HIV के साथ रहने वाला व्यक्ति effective ART से sustained viral load 200 copies/mL से नीचे maintain करता है तो वह HIV को sexually transmit नहीं करता। यह U=U का evidence-based principle है।

Condoms और other safer practices

Condoms HIV risk कम कर सकते हैं और कई other STIs तथा pregnancy risk को भी reduce करते हैं। Injection equipment share न करना और sterile equipment use करना blood-borne transmission prevention का important हिस्सा है।

Testing prevention का हिस्सा क्यों है?

HIV status जानना timely ART, PrEP/PEP decisions और partner protection में मदद करता है। Test type के अनुसार window period अलग होता है; recent exposure में PEP evaluation को testing के इंतजार में delay न करें।

When to seek prompt or emergency care

A possible HIV exposure within the past 72 hours needs urgent PEP assessment—the sooner, the better. Seek emergency care for severe breathing difficulty, confusion, collapse or another acute medical crisis. Testing and treatment should remain private, non-judgmental and clinician-led.
