HIV treatment: ART, viral suppression and U=U

Antiretroviral Therapy (ART) HIV treatment का foundation है। ART virus की replication को रोकती है, viral load को suppress करती है और immune system को recover करने का मौका देती है। Current guidelines HIV वाले सभी लोगों के लिए ART recommend करती हैं।

ART कब शुरू की जाती है?

HIV diagnosis confirm होने के बाद treatment को जितना जल्दी practical हो शुरू करना recommended है। Exact regimen patient की medical history, other medicines, kidney/liver health, pregnancy status और resistance-related factors के अनुसार clinician चुनता है।

ART कैसे काम करती है?

HIV medicines अलग-अलग stages पर virus को replicate होने से रोकती हैं। Modern regimens अक्सर combination treatment होते हैं, जिससे viral suppression मजबूत रहती है और resistance का risk कम होता है। किसी specific drug class या brand को खुद चुनना या बदलना उचित नहीं है।

Viral load और CD4 का क्या role है?

Viral load: Treatment response का मुख्य laboratory marker है। Goal sustained viral suppression है।
CD4 count: Immune status और कुछ opportunistic-infection decisions में useful हो सकता है। Monitoring frequency clinical situation पर निर्भर करती है।

U=U क्या है?

जब ART लेने वाला व्यक्ति sustained viral load 200 copies/mL से नीचे maintain करता है, तो वह HIV को sexually transmit नहीं करता। इसे Undetectable = Untransmittable (U=U) कहा जाता है। U=U treatment adherence और ongoing viral-load monitoring पर निर्भर करता है।

Adherence और missed doses

ART को prescribed schedule के अनुसार लेना जरूरी है। कभी dose miss हो जाए तो panic न करें और double-dose जैसे changes खुद न करें; regimen-specific advice के लिए clinician/pharmacist से पूछें। Repeated interruptions viral rebound और resistance का risk बढ़ा सकती हैं।

Ayurvedic herbs, supplements और ART

कोई Ayurvedic herb HIV को suppress करने या ART की जगह लेने का proven तरीका नहीं है। “Liver detox”, “CD4 booster” या “viral-load support” जैसे claims evidence-based HIV treatment नहीं हैं। Herbal products और supplements ART के metabolism को प्रभावित कर सकते हैं, इसलिए HIV clinician या pharmacist से interaction review कराए बिना उन्हें treatment plan में न जोड़ें।

When to seek prompt or emergency care

A possible HIV exposure within the past 72 hours needs urgent PEP assessment—the sooner, the better. Seek emergency care for severe breathing difficulty, confusion, collapse or another acute medical crisis. Testing and treatment should remain private, non-judgmental and clinician-led.
