जब HIV का इलाज नहीं होता या immune system काफी कमजोर हो जाता है, तो कुछ infections और cancers का risk बढ़ सकता है। इन्हें opportunistic infections (OIs) और AIDS-defining conditions कहा जाता है। आज effective Antiretroviral Therapy (ART) इन complications को रोकने का सबसे महत्वपूर्ण तरीका है।
Advanced HIV और AIDS कैसे define होते हैं?
HIV के advanced stage में CD4 count बहुत कम हो सकता है या कोई AIDS-defining illness हो सकती है। CD4 count एक useful marker है, लेकिन treatment decisions केवल एक number पर नहीं टिकते—viral load, symptoms, current medicines और दूसरी health conditions भी देखी जाती हैं।
महत्वपूर्ण opportunistic infections
- Tuberculosis (TB): HIV के साथ TB का risk बढ़ता है। Screening, preventive treatment when indicated, और active TB का prompt treatment बहुत महत्वपूर्ण है।
- Pneumocystis pneumonia (PCP): Severe immune suppression में होने वाला serious lung infection है। कुछ patients को CD4 level और clinical situation के आधार पर preventive medicine दी जाती है।
- Candidiasis: Oral thrush common हो सकता है; esophageal candidiasis जैसी severe disease medical evaluation मांगती है।
- Toxoplasmosis: Severe immune suppression में brain infection का risk हो सकता है; prevention और treatment clinician-directed होते हैं।
कुछ cancers का risk भी क्यों बढ़ता है?
Untreated or advanced HIV में Kaposi sarcoma और कुछ lymphomas जैसे cancers का risk बढ़ सकता है। ART से immune recovery होने पर कई HIV-related complications का risk कम होता है।
Prevention का practical framework
- ART नियमित लें: Viral suppression immune system को protect करने का foundation है।
- Regular medical follow-up: Viral load, CD4 और symptoms के अनुसार monitoring होती है।
- OI prophylaxis when indicated: PCP, toxoplasmosis या अन्य infections के लिए preventive medicines individual risk पर निर्भर करती हैं।
- Vaccination और screening: HIV care team age, immune status और medical history के अनुसार vaccines और screening plan बनाती है।
Ayurveda की सीमा: कोई Ayurvedic herb, supplement, bhasma या detox therapy HIV viral load को suppress करने, CD4 को reliably बढ़ाने या opportunistic infection को prevent/treat करने का substitute नहीं है। Herbal products ART से interact भी कर सकते हैं, इसलिए clinician/pharmacist review के बिना उन्हें HIV treatment के हिस्से के रूप में न जोड़ें।
